Tax Information: 1098-T
Overview
Per Internal Revenue Service (IRS) regulations, Pennsylvania College of Health Sciences must send the IRS Form 1098-T Tuition Statement to all eligible students, every year. The College is prohibited from offering any tax advice. All questions regarding Education Tax Credits must be addressed to a tax professional or the IRS at www.irs.gov.
The 1098-T Tuition Statement is processed each January and includes all eligible payments in box 1 up to the amount of qualified tuition and related expenses during the preceding tax year.

If you are a current student and have consented to receive your 1098-T electronically, you may access your form in The Nest by following these steps:
- Log onto The Nest
- Go to the Students tab
- Under Tuition Information
- Select Access your 1098-T Form
- Enter the tax year
If you are a recent graduate or not currently enrolled:
Log onto The Nest
- Enter your username and password
- If you cannot remember your password, click on the “Forgot/Reset Password” link under the LOGIN button and follow the instructions
- If you have graduated, you will still be able to reset your password for up to one year
From the home page, scroll down to Administrative Services
- Select the Student folder
- Go to the Students tab
- Under Tuition Information
- Select Access your 1098-T Form
- Enter the tax year
In an effort to maintain sustainability, 1098-T forms will not be mailed to your home.
If you require the form to be mailed, you can request it be mailed by emailing The Office of the Bursar at bursarsoffice@sju.edu or by calling our office at 610-660-2400. This form will be mailed to the Permanent Address on file. Please be sure your permanent address is correct. If you need to update your information please visit TeamDynamix and submit the request.

Explanation of 1098-T Fields:
- Box 1: Payments received for Qualified Tuition & Related Expenses Includes payments made with personal funds and educational loans as well as most scholarships and grants.
- Box 2: Qualified Tuition & Related Expenses billed Box previously reported amount billed in tax years prior to 2018. Note: Moving forward this box will appear blank.
- Box 3: This box was be checked for 2018 to indicate Pennsylvania College of Health Science changed from reporting QTRE billed to reporting QTRE paid in compliance with the IRS mandate. Note: Moving forward this box will appear blank.
- Box 4: Will reflect any applicable adjustments made to a previous academic year.
- Box 5: Scholarships, sponsored payments and grants credited to the student’s account during the 2018 calendar year regardless of the semester.
- Box 6: Adjustments to scholarships and grants that were reported in a prior tax year but were reduced in the current tax year.
Tax Identification Number
To comply with IRS regulations, Pennsylvania College of Health Sciences is required to collect your correct tax identifying number to file your 1098-T form with the IRS.
IRS regulations require you to provide your correct Social Security Number (SSN), or individual Taxpayer Identification (ITIN), if you are not eligible to obtain a SSN. If you fail to furnish your correct SSN or ITIN to the institution, then you are subject to a penalty of $50 unless your failure is due to reasonable causes and not to willful neglect.
If you did not provide the College with your SSN or ITIN during the admission process, please complete the form W-9S to provide your SSN or ITIN to the address listed in Part III on the form. Do not send this form to the IRS. If dropping this form off in person, please have your original SSN card and government issued picture ID with you. If mailing the form, also submit a notarized copy of your SSN card and government issued picture ID.
Due to security concerns, the form W-9S cannot be submitted by email.
